Palmitoylethanolamide (Normast®) versus NSAID in the treatment of TMJD pain
In a head to head comparison study the Italian research group from the Department of Orthodontics, of the  School of Dentistry, at the University of Bologna, could demonstrate that treatment with the natural analgesic compound palmitoylethalonamide (Normast®) was superior over the treatment with a classical NSAID in temporomandibular joint disorder (TMJD), or Costen syndrome.

25 patients affected by osteoarthritis and synovitis, who had been classified in Axis I, group III of the Research Diagnostic Criteria for TMD(RDC/TMD) entered a a blind randomized clinical trial. Group A(13 patientsw) were treated with palmitoylethanolamide (Normast, EpitechGroup, Padova) and Group B (12 patients ) were randomized to treatment with a NSAID(ibuprofen 600mg) 3 times a day for 2 weeks. Pain decrease after two weeks of treatment was significantly higher in group A (Normast ) than in group B (NSAID) (p=0.0001). And also the masticatory functions improved significantly more in group A than in Group B (p=0.0001).

Temporomandibular disorder (TMD) 

vas-pea.jpgTemporomandibular disorder(s) (TMD), or temporomandibular joint syndrome, is the most common cause of facial pain after toothache. In the past, many physicians called this condition TMJ disease or TMJ syndrome. TMD was previously known under the eponym of Costen syndrome, after Dr. James Costen. Costen described many aspects of the syndrome as it relates to dental malocclusion.

Today the term temporomandibular disorder (TMD) is the preferred term according to the International community. The National Institute of Dental and Craniofacial Research (NIDCR) puts TMJ and TMD together and refers to them as temporomandibular joint disorder (TMJD). Temporomandibular disorder(s) and treatment with palmitoylethanolamide (Normast®)

This study is quite important, as the treatment of TMD is not easy, and chronic NSAID treatment is often limited due to its negative side effects. Palmitoylethanolamide as a body own compound is easy to administer, has a very low side effect profile and can be prescribed to elderly patients without fear for negative interactions. Even combination therapy with pregabalin is possible and seems useful. [1]
